Join our team as a Personal Support Worker (PSW)!

Under the direction of the Co-Director of Care, the Personal Support Worker (PSW) is an integral part of the health care team, and in this role, encourages, supports and promotes independence for the residents by assisting with activities of daily living.

The Personal Support Worker (PSW) is responsible for a comprehensive range of duties encompassing resident care, interdisciplinary collaboration, adherence to organizational policies, and the maintenance of a safe environment. Core responsibilities involve provision of direct patient care, diligent observation and timely reporting of changes in health status, and accurate completion of all required documentation. 

We are looking for the following qualifications and traits:

• Must have successfully completed a personal support work program that meets the requirements in subsection 52. (2) of the Fixing Long Term Care Act, 2021 and Ontario Regulation ss. 246/22; and provides proof of graduation issued by the education provider

• Candidates are also considered who are enrolled in a RN or RPN program in Ontario

• Experience in Long Term Care (Gerontology) setting is an asset

• Ensures all Ministry of Long Term Care requirements are met

• Must demonstrate good communication skills

• Must demonstrate an active interest in ongoing education

• Displays a caring and respectful attitude toward our residents, families, and team members

• Understands the demands and rewards of the role

• To enhance the requirements for quality customer service and engagement

• Must be computer literate

Staff bring their authentic self to work each day and share their human experience with the people in their care. We are looking for the following qualifications and traits:

• Successful completion of or enrollment in Long Term Care Senior Management or equivalent 

• Passionate about dementia care

• Current with dementia best practice and evidence

• Visionary and inspirational

• Demonstrated Political acumen 

• Education in an emotion-based model of care an asset

• High levels of emotional intelligence

• Experience in long-term care and/or dementia care 

• 5 years of experience in a formal leadership position

• Project Management training and experience as a lead in a significant project

• Change Management certification and experience in initiating a significant change

• Post-secondary education in research and numerology

• Facilitator training and experience as a facilitator and educator

• Excellent communication skills – both written and verbal

• People management including mentoring, coaching, performance management, hiring and termination

• Knowledge and aptitude in Google Suite

• Drivers license and access to a vehicle

• Qualification and experience in formalized change management. 

• Demonstrated knowledge and experience utilizing project management methodology. 

• 2 years experience in Long Term Care (Gerontology) preferred 

• Must demonstrate an active interest in ongoing education 

• Must be computer literate

Reporting to the Director of Care, the role of a Registered Practical Nurse (RPN) encompasses a wide range of professional nursing responsibilities in accordance with the standards of nursing practice as outlined by the College of Nurses. Under the direction of the Registered Nurse, this role provides support in the coordination of care, overseeing designated nursing personnel, and executing tasks as delegated. The Registered Practical Nurse (RPN) is responsible for delivering direct care in accordance with the established care plans, ensuring the accurate and timely documentation of resident information, facilitating the orientation of new team members and residents, and participating in assigned committees. This position also requires the application of quality improvement and risk management principles, as well as the efficient management of supplies and the meticulous upkeep of the physical environment and equipment.

We are looking for the following qualifications and traits:

• Candidates must possess a current Certificate of Registration with the College of Nurses of Ontario. O. Reg. 246/22, s. 51

• Experience in a Long-Term Care (Gerontology) setting is an asset

• Performs care planning and coordination of care within a diverse multidisciplinary care team

• A demonstrated use of current care technology

• Shall exhibit strong leadership, communication and interpersonal skills

• Ensures all MOH and MLTC requirements are met

• Must demonstrate good communication skills

• Must demonstrate an active interest in ongoing education, training, and professional development

• Must be computer literate
